Identity theft and identity fraud are terms used to refer to all types of crime in which someone wrongfully obtains and uses another person's personal information in some way that involves fraud or deception, typically for economic gain.
If you believe you are the victim of identity theft that has resulted in the creation of a DIRECTV account using your identity without your authorization please send a completed and notarized affidavit to:

Once the affidavit has been received, further documentation may be required, including but not limited to:
- A police report for identity theft
- Proof of your address for the time frame in question
- Proof of your identity
